It assesses the environment a company already has, designs the target environment, and runs the move between the two. In practice that covers servers, networks, cloud, backup and security, from the first assessment through to day-to-day operation.
With an assessment of the environment you have today: servers, networks, licenses, backups and single points of failure. That is where the target architecture, the migration plan and the disaster recovery plan come from. Without it, any proposal is guesswork.
Support fixes the problem that showed up. Consulting deals with why problems keep showing up: the wrong sizing, an architecture that never kept pace with growth, no redundancy, no recovery plan. One answers the ticket, the other reduces how many tickets there are.

No. The in-house team knows the business and the systems, which no consultant has on day one. The consultancy brings experience in projects a company runs once every few years, which is exactly why nobody builds up that repertoire internally.
It depends on the scope. The assessment is usually short, and it is what makes estimating the rest possible with any precision. Migration projects run in phases, and the time for each one comes out of the assessment, not out of a generic table.
